Carers’ Support Worker Michele Glanfield is based at our Dewerstone Practice on Tuesdays and Wednesdays, to help carers obtain the help, support and information they need.
If you are a carer of any age, and are looking after a relative or friend, and need advice and support, please contact Michele.

Carers Health & Wellbeing Check
A Carer’s Assessment, known as a Health & Wellbeing Check, is a conversation about your caring situation and how it effects you. The assessment looks at all aspects of caring including your practical and emotional needs which might include having a short break, leisure activities, arranging sitting services or attending a support group. If needed, a further assessment can be arranged for your cared for.

Once you have completed the Health & Wellbeing Check booklet, please return it either to Michele at Chatto Road Surgery, or to Carers Aid Torbay.
