Register as a new patient

You can register with the Practice in person at any time during surgery opening hours.  Please bring with you some photo identification such as a photo driving licence or passport.  Photo ID is necessary for security reasons to ensure that access to your records and medication is only issued to you. Registration forms are available from reception.  You will be encouraged to attend for a New Patient Check with the Healthcare Assistant.  This is an important first contact for your new surgery.  Please bring a urine sample with you.

Please print and fill these forms in and hand them into reception with photo ID. Please allow a week to 10 days for registration to be completed after you hand in your forms to us.
